Whitecap Health Centre Medical Transportation Van Service

Medical Transportation Van Service is available to Whitecap Dakota First Nation members and residents for medical appointments. It is funded through Indigenous Services Canada (First Nations and Inuit Health Benefits).

Client's Responsibilities:

  • All bookings must be made by calling the Medical Transportation Coordinator at the Whitecap Health Centre between the hours of 8 a.m. to 4:30 p.m., Monday through Friday. Calling the Medical Van Driver is not accepted.
  • Clients must notify the Medical Transportation Coordinator as soon as you know of an upcoming appointment.
  • All trips will be scheduled accordingly. Due to Covid-19, there are no shared trips with other households currently. Otherwise, clients will be required to share the medical van with other clients as schedules.
  • Clients must remember when their appointment is scheduled and be ready when the Medical Van arrives at their home.
  • Clients must inform the Medical Transportation Coordinator a minimum of on day prior to their appointment if they are cancelling and no longer require medical transportation. Suspension from the Medical Van Service may occur after last minute cancellations or no shows.
  • Clients must stay at the location of the medical appointment until the Medical Van returns to the location to pick up the client.
  • Clients must contact the Medical Transportation Coordinator or the Medical Van Driver if the client is expected to be delated past the established time of pick up.
  • Clients are expected to have doctor's sign all required documents given.

Private Vehicle:

  • All Private Vehicle requests must be approved in advance of the appointment by the Medical Transportation Coordinator.
  • Due to Covid-19, clients are encouraged to utilize the Private Vehicle option.
  • Clients will be given a blue Certificate of Medical Attendance slip (CMA) and Gas Card of $16.00 (Indigenous Service Canada rate).
  • Clients are urged to bring back the signed blue CMA to the Whitecap Health Centre promptly before another CMA/Gas Card can be issued.
  • Clients can call the Whitecap Health Centre to plan to pick up or drop off the signed CMA.
  • If the Medical Van Service, Clients will be given the option for the Gas Card (Whitecap Trail Gas and Convenience Store).
