Salary Range:              $88K - $110K (dependent upon experience)

POSITION OVERVIEW

WDN is seeking a talented and motivated Home & Community Care Nurse to join our dynamic team. This role offers an opportunity to support the WDN Health & Social Development Department.

KEY DUTIES

The following duties are not intended to serve as a comprehensive list of all duties performed by all employees in this classification, only a representative summary of the primary duties and responsibilities. The employee may not be required to perform all duties listed and may be required to perform additional, position-specific duties.  In accordance with standards of WDN and reporting to the WDN Director of Health & Social Development, the Employee will:

Primary Responsibilities

  • Provide comprehensive case management services.
  • Conduct initial assessments and reassessments using approved assessment tools within program budget guidelines.
  • Interview members of the client’s support system as needed to gather relevant information.
  • Demonstrate strong interpersonal and written communication skills while maintaining objective, accurate, and timely client documentation.
  • Develop individualized care plans based on assessment findings and client/support system input.
  • Create client‑specific task lists with clear instructions for Home Health Aides, homemakers, or maintenance workers.
  • Initiate emergency services when required to protect client health and safety.
  • Collaborate with physicians and other health professionals or partner organizations to support coordinated client care.
  • Provide direct nursing care according to established care plans.
  • Monitor client progress, evaluate outcomes, and recommend changes to the care plan when appropriate.
  • Serve as a resource and liaison for clients and their families, supporting education and advocacy.
  • Maintain accurate records and complete month‑end reports as required.
  • Encourage and promote client independence through supportive decision‑making and care strategies.
  • Arrange ongoing personal care services when client needs exceed what can be safely provided by a Home Health Care Aide.

Supervisory Responsibilities

  • Supervise WDN Home Health Care Aides, ensuring high‑quality service delivery.
  • Provide initial and ongoing training for new staff as required.
  • Complete written evaluations of supervised staff and review results with the Clinical Integration Manager.
  • Assign client care tasks and review monthly schedules to ensure appropriate planning.
  • Review monthly performance statistics.

Program Development Responsibilities

  • Participate in the planning, development, and evaluation of Home Health Aide and Nursing Programs.
  • Serve on committees as assigned.
  • Support and facilitate ongoing staff education initiatives.
  • Contribute to program planning activities and quality‑improvement initiatives.

QUALIFICATIONS

  • Must be a Registered Nurse, currently registered, and in good standing with the CRNS.
  • Previous Home Care or Community Health experience.
  • Demonstrated supervisory, leadership, managerial and computer skills.
  • Demonstrated ability in public relations and interpersonal skills.
  • Excellent verbal and written communication skills, presentation, facilitation, planning and organizational skills.
  • Ability to work with minimal supervision, collaborate, interact, and build effective relationships at all levels of the organization.
  • Respect for privacy and confidentiality is essential.
  • Must possess and maintain a valid Saskatchewan driver's license and a reliable vehicle.

ADDITIONAL INFORMATION
