Home and Community Care Program

The Whitecap Home and Community Care Program is available to all residents of the community. The conductors of the homecare program are the patients, family, doctors and other allied health professionals and support workers. Patient and family participate in planning care, goal setting and informing healthcare providers with any changes in health, medication, or updated health advice from doctors.

The goals of the home care program are to promote and maintain individual health and independence of all Whitecap residents in a comfortable, safe home environment.

Access to services begins with a referral from the following: the patient, family members, friends/neighbors, hospitals, other health care workers, healthcare providers, and other community services or institutions.

The admission to homecare begins with the intake/referral process. The client is assessed using the Home and Community Care Assessment booklet to create a client profile, which includes health history, medication list, and assessment of activities of daily living at home. The client is also assessed for functional status for activities of daily living. Referrals are made to Occupational health if medical equipment is needed. Household members and immediate family members are included in the care plan, and their ability to assist in the care of the patient. The Home and Community Care Program does not replace the support and care that the family provides.

Support from outside services such as Client Patient Access Services (CPAS) are utilized when patient needs exceed the Home and Community Care resource capacity. The Whitecap Home and Community Care Program is available from 8:00 am to 4:30 pm Monday to Friday.

Examples of homecare eligibility:

  • Hospital discharge
  • Waiting for long term care placement
  • Have an illness or a disease that requires follow up
  • Need nursing care to maintain health
  • Choosing to live at home as long it is safe
  • Have an assessed need or disability
  • Family and friends who need support to continue to care for person at home
