Community Health Nurse

Community Health Nursing work is guided by circumstances, values, and the determinants of health. Whitecap Dakota has two health nurses who are committed to serving members with confidential, caring and professional care.  

Community Health Nurses (CHN) provide care to promote the health of individuals, families, groups, and the community. CHNs work in collaboration with clients, the client's family, Elders, Leadership, community groups and liaise with other health professionals. Community health nurses adhere to the standards and foundation competencies for the practice of registered nurses (SRNA) and the code of ethics (CNA).

The scope of practice includes the standards of health promotion, prevention and health protection, health maintenance, rehabilitation, and palliation.

  • Immunization for children and other age groups according the Saskatchewan immunization program
  • Communicable Disease control includes reportable diseases including the covid-19 pandemic
  • Maternal Child Health – Prenatal, postnatal, infancy and preschool
  • School aged health programming
  • Adult health promotion
  • Chronic Disease management (case management, referrals, linkages)
